Request for a Labor Organization Application / Annual Report
You must submit an application to obtain registration to operate as a labor union in Florida.
APPLICATION: Please submit Labor Organization Application/Annual Report. Complete the application by clicking on the "Printable Application" link at the bottom of the page.
FEES: You must submit a $1.00 filing fee in the form of a check or money order payable to the Department of Business and Professional Regulation. This fee is required annually with each application.
PERMITS: All permits expire at midnight, December 31.
ATTEST STATEMENT REQUIRED: You must sign the attest statement provided on the last page of the application acknowledging that you have answered application questions completely and truthfully to the best of your knowledge.
