Certificates of Operation (elevators, escalators and other vertical conveyances)

Once elevator construction is completed and the elevator company inspects and turns the conveyance over to the owner, the owner then must apply for the annual license to operate, also called a Certificate of Operation. The bureau will issue a Certificate of Operation annually upon receipt of a renewal application to the new owner. The owner is responsible for maintaining the license, annually scheduling inspections to ensure safe operation, and providing the bureau with current contact and mailing information.

Application Requirements:
 
  • CONTRACTED JURISDICTIONS: If your elevator operations are within one of the bureau's contracted jurisdictions: counties of Broward and Miami-Dade, cities of Miami and Miami Beach and Reedy Creek Improvement District; please contact the appropriate jurisdiction for permits, licensing, inspections, and other services provided.

  • INSPECTION: Unless exempt by law, all elevators, escalators and other vertical conveyances must pass an annual inspection with no violations before the bureau can issue a Certificate of Operation. A current, satisfactory inspection performed by a certified, private, third-party inspector is required for initial licensure and each annual renewal. Two-Stop Elevator Inspection Exemption - Florida law exempts two-stop elevators with a current full-service maintenance contract from the annual inspection requirement. You must notify the bureau of the service maintenance contract if you believe you qualify for this exemption. For more information on inspection requirements, see our elevator website.

  • APPLICATION: Obtain and complete DBPR HR-7025 Application for Certificate of Operation or Change of Owner in Word or Fill-in PDF (also referenced at the bottom of this page). A separate application is required for each conveyance. The application includes instructions.


  • LICENSE FEE: The annual license fee for elevators is $75. This fee must be paid before a license will be issued. Payment must be post-marked by July 31st of each year, or a late fee will be added. Please make checks payable to the Division of Hotels and Restaurants. For faster processing, please write the license number of the elevator on the check. If you own more than one elevator, you may write one check for all and enclose of all your elevator license numbers with the combined check. If the elevator company paid the initial license fee with the permit, the owner will not be charged a fee for the first-year license, but is responsible for subsequent annual renewals and fees.

    A license fee is not required for a change of owner application. If a license fee is due to renew the certificate of operation, the new owner can submit the outstanding license fee along with a change owner application.
