This form is used by individuals who are requesting the Department of Business and Professional Regulation to reinstate their null and void license. Applicants must demonstrate to the Department that they failed to timely renew their license due to an economic hardship or illness. If the Department denies your application, you will receive notification from the Department outlining your hearing rights. If your application denial is upheld, you will be required to apply for a new license, which may include retaking an examination.
DEFINITIONS: A license becomes "null and void" after a licensee fails to renew the license for two consecutive licensure cycles. A null and void license cannot be reinstated unless the applicant demonstrates to the Department that he or she failed to renew the license due to an illness or economic hardship. There is no specific definition for "illness" or "economic hardship." The Department reviews each application on a case-by-case basis.
