Asbestos Consultant - Licensure from Null and Void (ALU 9)
This form is required if you are applying to reinstate an asbestos consultant license that is in null and void status. This application should only be used by persons who have previously held a license as an asbestos consultant. You should only apply for the same type of license previously held.
Application Requirements:
CONTINUING EDUCATION:Provide proof of completion of continuing education requirement.
EXPLANATION:Provide an explanation of the illness or economic hardship that prevented renewal of your license.
FEE:Pay the required fee as provided in the application. If applying online, you can pay with a credit card or electronic check. If mailing your application, please include a check payable to the Department of Business and Professional Regulation. Military veterans, their spouses and Florida National Guard members may be eligible for a fee waiver, fee reimbursement, and/or fee discount.
