Professional Individual Occupational License - Initial Online Submission

Positions include, but are not limited to: racing animal owner (greyhound, harness, quarter horse or thoroughbred), authorized agent, jockey agent, racing or game official, or alternate, association officer, director, manager, assistant, chief of security, doctor, general manager, harness driver, horse broker, bloodstock agent, horseman's bookkeeper, jockey or apprentice jockey, nurse, officer/ director/ shareholder of any business, official (race or game), paramedic, EMT/ physicians assistant/ plant or track superintendent, trainer or assistant trainer, player (jai alai), public relations director, veterinarian, mutuels manager, totalisator manager/supervisor, assistant moneyroom manager supervisor, assistant and or any other professional person with access to restricted areas.

Application Requirements:
  • APPLICATION: Individuals applying for the first time must complete a DBPR PMW-3120 Individual Occupational License Application form. Complete the application by clicking on the "Printable Application" or "Online Services" link at the bottom of the page.
  • PROFESSIONALS INCLUDING: doctor, nurse, paramedic, emergency medial technician (EMT), physician's assistants, and veterinarian (including track veterinarian) must have a Florida professional license with the proper state agency, prior to occupational licensure by the Division of Pari-Mutuel Wagering.

  • LICENSE TERM: Florida Pari-Mutuel licenses are valid from July 1st and shall be valid for a period of three fiscal years.

  • FEE: The license fee is $80 for a three-year license.

  • FINGERPRINTS: Initial applicants must submit a complete set of fingerprints to the Division either on a FD-258 fingerprint card or via a Livescan device. Fingerprints will be electronically retained for future submissions to the FDLE for those individuals timely renewing their license. Every 3 years thereafter, upon renewal, the applicant must pay a resubmission fee for his/her fingerprints to be resubmitted to the Florida Dept. of Law Enforcement for an updated criminal history background check.   • Electronic Fingerprints (Required for Online Applicants): Electronic fingerprints must be submitted to the Division through a law enforcement agency or an FDLE approved Livescan Service Provider. Submit electronic fingerprints with the following ORI number: FL920630Z. You must provide your Transaction Control Number (TCN) with your application. Note: Electronic processing fees must be paid directly to the law enforcement agency or FDLE approved Livescan Service Provider.

  • Fingerprint Card: Completed fingerprint cards (FD-258) must be mailed to the Division with your application and the $37.25 fingerprint processing fee.

  • Fingerprint Resubmission (Renewal Applicants Only): Applicants timely renewing their license need only provide the Division the $13.25 fingerprint resubmission processing fee with their application. Note: Timely submission of renewals must occur within one year of the expiration of the applicant’s license.

  • Exemptions from Fingerprinting: An applicant who is under 18 years of age is exempt from fingerprint requirements; and instead of a fingerprint card, an applicant who is 70 years of age or older submits $24 for a background information records check through the Florida Department of Law Enforcement.

  • VERY IMPORTANT FOR ONLINE APPLICANTS: If you have previously been issued a license by the Division of Pari-Mutuel Wagering, you must link your license to your online account. If eligible for renewal, you will be able to renew your license utilizing DBPR Online Services. You may verify your license number by clicking the link below and search by entering your name.

  • NOTE: Certified law enforcement or corrections officers working in security or safety positions do not require an occupational license, except when working as the Chief of Security.;

    Fees may be paid by check or money order only and should be made payable to DBPR in U.S. funds. Application packages may be submitted at a PMW field office or mailed to the following address:
