Slot Machine Business Employee License - Renewal

For general- and professional-level employees who work for a business entity and need access to a slot machine facility in Florida.

Application Requirements:
 
  • APPLICATION: A DBPR PMW-3410 Individual Slot Machine Occupational License Application form must be completed. Complete the application by clicking on the "Printable Application" or "Online Services" link at the bottom of the page.

  • LICENSE TERM: Florida Slot Machine Licenses are valid from May 1st through June 30th of the following year.
  • FEE: Application fee is $50 for a one-year license, $100 for a three-year license. Payment can be made by check or money order made payable to DBPR. An honorably discharged disabled veteran from one of the time periods specified in s.1.01(14), F.S., or the unremarried spouse of a deceased disabled veteran meeting this criteria, may be exempt from license fees up to $50.00. Fingerprint fees are not exempt.

  • IDENTIFICATION: Two forms of identification as required by Rule 14.010, Florida Administrative Code 61D-14.010.
  • AUTHORIZATION FOR RELEASE OF INFORMATION: must be completed PMW-3460.
  • AGE REQUIREMENT: Applicants for Slot Machine licensing must be 21 years of age or older.
  • FINGERPRINTS: Fingerprints are due upon initial licensure and every three years thereafter. Fingerprints will be electronically retained for future submissions to the FDLE. Every three (3) years after the initial fingerprint submission, upon renewal, the applicant must pay a $14.75 for his/her fingerprints to be resubmitted to the Florida Dept. of Law Enforcement for an updated criminal history background check.
