Community Association Manager - Set License to Active (CAM 4)

This form is used to reactivate a license outside the renewal cycle.

Application Requirements:
  • CONTINUING EDUCATION: Complete the required 15 hours of continuing education from an approved provider. Submit course certification(s) with application.
  • FEE: Submit $25 with the course completion certificates (payable to the Department of Business and Professional Regulation).
  • APPLICATION: Complete the application by clicking on the "Printable Application" link at the bottom of the page.
