Asbestos Business - Financially Responsible Officer (ALU 5)

This application is used by entities that wish to change their financial officer. The financial officer must sign all checks, drafts or payments made by the business and has authority to act for the business in all financial matters.

Application Requirements:
 
  • INSURANCE: You must have workers' compensation insurance or an exemption. You must also have public liability insurance as set forth in the requirements listed in the application.
  • FINANCIAL/CREDIT: An applicant shall be required to forward a credit report, financial statement, and either a statement of bond ability or irrevocable letter of credit to the department.

  • FEE: Submit the $200 fee (payable to the Department of Business and Professional Regulation).

  • APPLICATION: Complete the application by clicking on the "Printable Application" link at the bottom of the page.
