Personal Address Change-Sales Associate or Broker Sales Associate (Form RE 10)
This transaction is used to notify the department of changes to the address of the licensee.

PLEASE NOTE: The application has two address change options to choose from. One option is for address changes without requesting issuance of an updated license and the other is for address changes with issuance of an updated license. Please be sure to select the option most appropriate for your needs.
