Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

08B-49-4    Basic - Employee personal food not properly identified and segregated from food to be served to the public. Employee food in walk in cooler not separated from customer food.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee drinks above reach in cooler.
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. 2 employees working without hair restraints when inspector wakes in. Employees placed hats on head during inspection **Corrected On-Site**
36-73-4    Basic - Floor soiled/has accumulation of debris. Floors and walls soiled. Throughout the prep kitchen line.
08B-38-4    Basic - Food stored on floor. Container of rice and container of frying oil stored on floor in kitchen by freezer. Container of sauce and container of water and lemons stored on floor by hand wash sink. **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Handwashing sink handles, and soap dispenser very soiled. **Repeat Violation**
25-32-4    Basic - Reuse of single-service or single-use articles. Several containers of Kikkoman soy sauce being reused for food storage.
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Bags of chicken wings being left out on counter by triple sink thawing. Large tub of pork in standing water thawing. Discussed with manager. She moved product to properly thaw. **Corrected On-Site**
11-04-5    High Priority - Certified Food Manager or person in charge does not require employees to report illnesses and symptoms as required. Owner could not provide proof of employee reporting of illnesses.
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Dole pineapple juice 1 qt Lee Kum Kee Panda Brand Oyster Sauce 3 .5 cans Koon chun ground bean sauce .5
12A-16-4    High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Employee came in from outside and grabbed unwrapped food from reach in cooler, no hand wash. Discussed with management the importance of hand washing. Manager discussed with employee.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. 1 live fly in kitchen. 2 live flies in back of kitchen
50-17-3    High Priority - Operating with an expired Division of Hotels and Restaurants license. Operator has 2024 expired license. **Admin Complaint**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Approximately 150 rodent droppings in the bathroom being used for food storage.
25-31-5    High Priority - Single-use take-home food container refilled with food. Lo mein plastic bags being used to store fried shrimp.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Cutting boards soiled on cook line. Walk in cooler shelves soiled.
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Bottle of honey and telephone stored on hand wash sink. Manager moved. **Corrected On-Site**
51-16-7    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. The direct link to the Plan Review page is http://www.myfloridalicense.com/DBPR/hotels-restaurants/licensing/plan-review/. Men's bathroom on plans being used for food storage, to include a freezer chest. Upon review of plans, plans do not reflect men's bathroom being changed to a food storage area. Operator will need to submit a new plan.
