Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-36-4    Basic - Ceiling tile missing. Ceiling tiles missing in back kitchen over walk in cooler, also at hallway with silver reach in cooler. Floors with cracked tiles where fryer is located, around drain in cookline. Standing water on floor in bar area.
35A-03-4    Basic - Dead roaches on premises. One dead roach observed in right sided oven. One dead roach observed on ground near silver double door reach in freezer. **Repeat Violation**
14-11-5    Basic - Equipment in poor repair. Observed rusty screw on right side of ice machine.
25-11-5    Basic - Single-service articles stored outside or in a room/shed that is not fully enclosed (open to outdoors/screened). Opened containers of single service items stored outside back door in hallway that opens to outside. Not fully enclosed.
29-19-4    Basic - Standing water in floor drain/floor drain draining very slowly. Standing water in dishmachine floor drain. May overflow if in use. Person in charge fixed during inspection. **Corrected On-Site**
08B-12-5    Basic - Stored food not covered. Taco shells in bus tubs on shelf in kitchen uncovered. Chile rellenos in reach in freezer uncovered. **Repeat Violation**
22-41-4    High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishmachine at front line testing at 0ppm. Person in charge will utilize triple sink. **Corrective Action Taken**
12A-12-4    High Priority - Employee switched from working with raw food to ready-to-eat food without washing hands. Observed employee handle raw steak and place on grill with gloves hands, then handle container of ready to eat seasoning with same gloves. 08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Raw chicken stored over raw pork in walk in cooler, person in charge moved to appropriate location. **Corrected On-Site**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Salsa from reach in cooler on ice at server station less than 2 hours ago at 47F. Person in charge added ice around container. Pork sitting out in container in kitchen brought from walk in cooler per person in charge less than 1 hour ago at 63F. Beans held in pan on shelf above cookline at 100F. Person in charge states brought out of walk in cooler to utilize on line. Out less than 4 hours. Person in charge moved to reach in cooler. **Corrective Action Taken** **Repeat Violation** **Admin Complaint**
41-10-4    High Priority - Toxic substance/chemical improperly stored. Chemical sprays stored with liquor bottles in office. Chemical spray stored in server area with food products, stored on shelf with nozzle facing condiment bottles at server area. In back hallway pesticide stored with drink mixes. Person in charge began moving to appropriate location.
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Hand wash sink not accessible to employees at far cookline due to leaking onto ground. Person in charge added catch bucket during inspection. Fixed far cookline during inspection. **Corrective Action Taken**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Ice observed in server line hand wash sink. **Repeat Violation**
27-18-4    Intermediate - Hot water supply not maintained during peak periods. Water for establishment at 78F during inspection. Working with contractor to repair. **Repeat Violation** **Warning**
05-12-5    Intermediate - Probe thermometer not used to ensure proper food temperatures of received time/temperature control for safety food. Observed employee accept food delivery without taking temperatures of time/temperature control for safety food products
