best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.

High Priority - Roach activity present as evidenced by live roaches found.
1. One live roach observed crawling along wall next to reach in chest freezer in prep area.
2. Approximately three live roaches observed coming from door jamb in dry storage area.
3. Approximately two live roaches observed in wall opening at wall next to triple sink in prep area. **Warning**
Intermediate - No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Provided operator time as a public health form . **Warning**
