A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.

In kitchen on top of reach in cooler 6 live roaches. Operator killed, cleaned and sanitized cooler.
In kitchen on floor 1 live roaches found walking in front of fryer. Operator killed and cleaned and sanitized.
In kitchen under wok cooking station 1 live roach on floor operator killed cleaned and sanitized.
In kitchen dry storage area next to cook line approximately 30+ live roaches crawling on boxes containing seasonings and sauces. Operator began removing boxes and killing roaches.
In kitchen behind reach in freezer approximately 4 live roaches walking on ground.
In bathroom in lobby 1 live roach operator killed and sanitized.
In customer dining area 1 live roach crawling on floor by front counter. Operator killed cleaned and sanitized.
In customer waiting area 1 live roach crawling on trash can operator killed cleaned and sanitized. **Warning**
High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit.
At cook line cooked rice (55F - Cold Holding) per operator product not portioned or prepared today. Operator states they leave rice there under no temperature control for rush hour until they cook. Per operator product removed from cooler approximately 1 hour ago. Operator placed rice back into walk in cooler approximately 1 hour ago.
