A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin.
Soiled with black mold like substance and lime scale build up.
Bar area entrance soda machine ice bin interior heavily soiled with black mold like substance build up. **Warning**
Basic - Current Hotel and Restaurant license not displayed.
License posted in dining room area expired on 2/1/2022 and has the old ownership name as ARC WingHouse LLC. **Warning**
Basic - Dead roaches on premises.
2 dead roaches observed on floor next to walk in cooler door near kitchen back entrance.
27 dead roaches were observed on the floor beneath the triple-door freezer, the freezer chests, racks storing single service items, bags of batter mix, sauce bottles, and chemicals throughout the outdoor storage area.
Employee is cleaning and sanitizing the area. **Repeat Violation** **Warning**
Basic - Floor area(s) covered with standing water.
Soiled standing water was observed under the reach in deli cooler and in the walk in cooler, caused by missing and cracked floor tiles. **Warning**
Basic - Floor soiled/has accumulation of debris.
Floors soiled with heavily food debris and grease build up under fryers and coolers on cook line. **Warning**
Basic - Floor tiles missing and/or in disrepair.
Missing and cracked floor tiles were observed beneath the reach in deli cooler and flat top grill on the cook line, as well as in the walk in cooler. **Warning**
Basic - Hole in or other damage to wall.
A hole was observed in the wall next to the plumbing pipes behind the prep table across from the dish machine. The wall panel appeared to be in disrepair and is falling off. The manager stated that this was due to a recent plumbing leak. **Warning**
Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris.
Conventional and pizza oven interiors soiled with heavy food debris build up. **Warning**
Basic - Reach-in cooler interior/shelves have accumulation of soil residues.
Interior of reach in coolers across from conventional oven and flat top grill soiled with dried up food debris.

High Priority - Live, small flying insects found
Observed 1 live fly flying under prep table across from dish machine area.
Observed 2 live flies flying above dish machine area.
Did not observe flies landing on any food or food contact surfaces. **Repeat Violation** **Warning**
High Priority - Roach activity present as evidenced by live roaches found.
1 live roach observed crawling on floor under reach in cooler across from conventional oven. **Repeat Violation** **Warning**
Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime.
Cutting boards stained on reach in deli coolers.
Can opener, slicer, and vegetable dicer soiled with dried up food debris. **Warning**
Intermediate - No paper towels or mechanical hand drying device provided at handwash sink.
Missing at hand sink next to flat top grill. Employee placed a roll of paper towels at the hand sink. **Corrected On-Site** **Warning**
