A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Dead roaches on premises.
Approximately 2 dead roaches In kitchen on floor behind reach in cooler.
Approximately 7 dead roaches on glue traps in kitchen under cook line.
Approximately 1 dead roach on floor in kitchen dry storage room.

High Priority - Live, small flying insects found
Approximately 10 small flying insects in kitchen landing on food preparation table.
Approximately 20 small flying insects inside kitchen next to walk in cooler and ice machine landing on walls, exterior of ice machine and ceiling.
Approximately 15 small flying insects in kitchen landing on dishwashing machine and walls.
Approximately 15 small flying inside kitchen at cook line landing on shelves, walls and ceiling.
Approximately 30 small live flying insects at bar area landing on hand washing sink, soiled cloth, and bar top.
Discussed with operator to eliminate all flying insects and clean and sanitize all surfaces.

High Priority - Roach activity present as evidenced by live roaches found.
Approximately 1 live roach in kitchen walking on floor under dish washer.
Discussed with operator who killed roaches found, and to clean and sanitize areas. **Warning**
Intermediate - Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection.
In reach in cooler cut melon (46F - Cooling) at 3:25 since 4:25 to 46F at 3:40, at this current rate of cooling product will not reach 41F within 4 hours, discussed with operator who moved product to freezer to chill. **Corrective Action Taken** **Warning**
