A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Discussed with operator to clean and sanitize ice machine. **Warning**
Basic - Food stored on floor.
In non functional walk in cooler sealed bags of sugar stored on floor, discussed with operator to remove bags of sugar from floor. **Warning**
High Priority - Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours.
In reach in cooler large plastic bin of cooked bean soup (47F - Cooling)at 11:05 per operator product prepared yesterday evening and place in reach in to cool, at this rate of cooling product never reached 41F within 6 hours see stop sale. **Repeat Violation** **Admin Complaint**
High Priority - Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands.
Employee handling dirty dishes did not wash hands before handling clean and sanitized dishes, discussed with operator employee washed hands. **Corrected On-Site** **Warning**
High Priority - Live, small flying insects found
At bar approximately 1 live small flying insect landing on bar top and tip jar, discussed with operator to eliminate flying insect and clean and sanitize areas. **Warning**
High Priority - Roach activity present as evidenced by live roaches found.
In kitchen approximately 30 live roaches inside non working walk in cooler crawling on food debris on floor, shelves used to store clean and sanitized dishes, and walls of walk in cooler, discussed with operator to eliminate all roaches and clean and sanitize dishes and area. **Warning**
High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled.
Employee wearing gloves handling raw fish did not wash hands and change gloves before handling cut avocados, discussed with operator employee washed hands and changed gloves. **Corrected On-Site** **Repeat Violation** **Admin Complaint**
High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse.

At three compartment sink bucket containing soap water stored directly next to raw fish, discussed with operator who removed soap bucket.
In walk in cooler pesticide tracking powder stored directly next to and with clean and sanitized dishes, discussed with operator who removed product. **Corrected On-Site** **Warning**
High Priority - Tracking powder pesticide used inside establishment.
Inside of walk in cooler roach tracking powder stored on shelf, being used on floor in walk in cooler and behind the bar area on floor, discussed with operator to clean and sanitize all areas. **Warning**
High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb.
Exterior hose bibs in rear of store have no vacuum breaker installed, discussed with operator install vacuum breaker. **Warning**
Intermediate - Handwash sink used for purposes other than handwashing.
