Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-65-5    Basic - Ventilation inadequate as evidenced by excessive steam or vapors in kitchen. Observed inadequate ventilation at the time of the inspection. The hood system is not working above the fryer. **Warning**
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. Observed both mops inside the bucket. Employee removed them. **Corrected On-Site** **Warning**
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed at the kitchen area and sushi counter. **Repeat Violation** **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 2 live roaches crawling inside the reach in cooler door gaskets across from the stove. **Warning**
03F-02-5    High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control can be determined. Observed rice not time marked but rest opened at 3:00 pm **Warning**
16-37-1    Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. **Warning**
53A-05-6    Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. 03F-10-5    Intermediate - No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Form was provided to operator. **Warning**
53B-13-5    Intermediate - Proof of required state approved employee training not available for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed establishment has 15 employees working but they just provided food handler training for 6 employees. **Warning**
45-04-4    Use of cooking equipment producing grease laden vapors/smoke with no hood suppression system installed. Notified Fire AHJ. For reporting purposes only. Observed the hood suppression system is not working. The kitchen has 3 hoods and 1 of them is not working.
