Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

22-20-5    Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin.
23-24-4    Basic - Buildup of food debris/soil residue on equipment door handles. Observed on walk in cooler handle.
24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed ice bucket not inverted next to ice machine.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed personal items on rack with to go supplies next to ice machine.
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Observed employee engaging food preparation not wearing a hair restraint.
24-08-4    Basic - Equipment and utensils not properly air-dried - wet nesting. Observed on clean metal containers next to 3 compartment sink.
36-22-4    Basic - Floor area(s) covered with standing water. Observed by walk in freezer door entrance.
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed grease accumulated under fryers.
14-69-4    Basic - Ice buildup in walk-in freezer.
10-02-4    Basic - In-use utensil not stored on a clean portion of food preparation or cooking equipment. Observed ton at front counter next, employee removed ton to dishwashing area. **Corrective Action Taken**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed soiled gaskets on biscuits reach in cooler, also on reach in freezer next to breaded station.
33-16-4    Basic - Open dumpster lid.
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed soiled interior biscuits reach in cooler. Also observed mold-like substance on reach in cooler interior at sandwich assembly area.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed soiled wall at breaded station.
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry.
12A-07-5    High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed operator changing gloves without washing their hands. Coached employee on proper procedures.
12A-09-4    High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee changing tasks and not changing gloves as needed.
31B-05-4    Intermediate - Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Observed across biscuits oven.
