Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

14-01-5    Basic - Bowl or other container with no handle used to dispense food. Observed scoop with no handle being used to scoop dry food also inside the Parmesan cheese at the prep table at the kitchen. **Warning**
36-36-4    Basic - Ceiling tile missing. Observed several ceiling tiles missing in the dry storage area. **Warning**
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed vents soiled in the front area above prep table. **Warning**
24-26-4    Basic - Clean equipment/utensils not stored at least 6 inches above the floor. Observed cleaned food containers and utensils stored on the floor at the ware wash area. **Warning**
24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed food Plastic containers and glass bottles stored upright at the dry storage area. **Warning**
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Observed cutting boards with deep grooves in front of prep area and in the kitchen. **Warning**
12B-02-4    Basic - Employee eating in a food preparation or other restricted area. Observed female chef drinking coffee over prep table kitchen area. **Warning**
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed personal water bottles stored on the waiter prep counter. **Warning**
13-07-4    Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed female employee wearing a bracelet at in front counter area while preparing food. **Warning**
14-11-5    Basic - Equipment in poor repair. Observed door gaskets of reach in cooler across of stove and walk in cooler door gaskets in disrepair. **Warning**
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Observed at the back exit door. **Warning**
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed food debris all around walk in freezer floor. **Warning**
08B-38-4    Basic - Food stored on floor. Observed containers of dried beans, sugar and granola stored directly in the floor in the dry storage area. Also observed bread and ice cream stored in the floor inside walk in freezer. **Repeat Violation** **Warning**
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Observed several tongs hung on oven handles in the kitchen. **Repeat Violation** **Warning**
10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed scoops handled in contact with corn starch, beans, sugar and flour at the dry storage area. **Warning**
10-17-4    Basic - In-use utensil stored in cracks between pieces of equipment. Observed spatulas stored in between wall and hand-wash basin by preparation area. **Warning**
02D-03-4    Basic - Label on a food item prepared and packaged onsite for customer self-service does not include product identity/description, date product was packaged and name and address of the establishment that prepared and packaged the food. Observed in house made, packed fruits, granola, sauces, bottled juice shots, overnight oats and chia pudding, all available for grab and go, missing address of establishment and date marking. **Warning**
51-13-4    Basic - No Heimlich maneuver/choking sign posted. **Warning**
05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. Observed no thermometer in RIC across of stove. **Warning**
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Observed no handwash signs in all the handwash sinks in kitchen area and also inside one of the bathrooms. **Warning**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler and walk in cooler door gaskets soiled. **Warning**
16-46-4    Basic - Old labels stuck to food containers after cleaning. Observed on plastic containers stored at warewash area above sink. **Warning**
24-18-4    Basic - Silverware/utensils stored upright with the food-contact surface up. Observed spoons improperly stored at waiter station at front counter. **Warning**
21-10-4    Basic - Soiled dry wiping cloth in use. Observed soiled dry cloth on prep cutting boards in kitchen area. **Warning**
14-12-4    Basic - Utensils in poor condition. Observed burned silicone and plastic spatula heads in the utensil container in kitchen area. **Warning**
36-30-4    Basic - Wall not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom. Observed brick wall not smooth at the dry storage area. **Warning**
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloths not in sanitizing solution but stored on prep cutting boards in the kitchen area and dry storage area. **Repeat Violation** **Warning**
22-41-4    High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed a 0 ppm chlorine concentration when tested after running a wash. **Warning**
12A-05-4    High Priority - Employee used tobacco, ate, or drank then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed female chef after drinking coffee over a prep table in kitchen, proceed to handle food without washing her hands. Communicated to chef the correct procedures to avoid cross contamination. **Warning**
50-17-3    High Priority - Operating with an expired Division of Hotels and Restaurants license. Observed license expired 10/01/2025. Manager was able to pay for license before inspection was completed. **Corrected On-Site** **Repeat Violation** **Warning**
41-05-4    High Priority - Pesticide/insecticide labeled for household use only present in establishment. Observed a spray can of RAID stored on a shelf next to cooking oil in the dry storage area. **Warning**
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw fish stored above ready to eat cheese inside the walk in cooler. Also observed raw shelled eggs over ready to eat food in the reach in cooler at the kitchen. **Warning**
29-42-4    High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed vacuum breaker missing at mop sink splitter. **Warning**
53B-16-4    Intermediate - Employee has not received adequate training related to their assigned duties as evidenced by the inability to answer basic food safety questions. Observed employee designated to ware washing unable to answer the required concentration for quat and chlorine sanitizers. **Warning**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed cutting boards soiled in front prep area and in the kitchen. **Warning**
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Observed handwash sinks blocked with garbage bins at the front area and blocked with wheeled table at prep area. **Warning**
02B-01-5    Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Observed no asterisk on the menu indicating consumer advisory for poached eggs for the dishes Huevos Cazuels, Ahogados Poche, and Huevos Incrustados; and sunny side up eggs for the dishes Huevos Rancheros, Huevos Condesa, and seared tuna for the dish Atun Spicy. **Warning**
16-37-1    Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed no quaternary and chlorine test strips at the warewash area. **Warning**
53B-02-5    Intermediate - No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed the following employees with no food handler training for Yesenia, Gabriela, Disamar,Rafael, Alex, Candy, Carolina, Lizette, Catiela, Barbara. Per manager, all have been employed over 60 days. **Warning**
02D-06-5    Intermediate - Packaged food has no English labeling. Observed white and yellow powder substance in food containers not labeled in the dry storage area shelves. Observed bottles with liquids not labeled in the kitchen area above prep table. **Warning**
45-04-4    Use of cooking equipment producing grease laden vapors/smoke with no hood suppression system installed. Notified Fire AHJ. For reporting purposes only. Observed one fat fryer being used, stored on prep table at prep area with no hood suppression system above it.
