A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-01-4    Basic - Floor not cleaned when the least amount of food is exposed. In kitchen and Around all cooking and cold unit equipment soiled with visible food and dirt debris. **Warning**
08B-47-4    Basic - Food not stored at least 6 inches off of the floor. In kitchen at cook line - bucket of seasoning on floor. In back storage area - bag of bulgar wheat stored on floor . Educated operator on proper storage. **Warning**
10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. At cook In use utensils sitting in dirty water at 92F. Advised operator on proper storage of utensils. **Repeat Violation** **Warning**

12A-20-4    High Priority - Employee washed hands with no soap. Employee came from outside rinsed without soap and began preparing food without a proper hand wash. **Warning**
35A-02-7    High Priority - Live, small flying insects found Approximately 10 plus small flying insects flying around front counter landing on shelving ,guest seating tables and around the triple sink dish wash area. **Warning**
14-15-4    High Priority - Nonfood-grade containers used for food storage - direct contact with food. At walk in cooler raw goat a raw beef stored in trash bags. **Warning**
08A-17-6    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Walk in freezer - raw salmon over raw liver. **Warning**
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. At walk in cooler Raw chicken (52F - Cold Holding); Cooked fish (53F - Cold Holding); Cooked pork (55F - Cold Holding); Cooked pig tails (47F - Cold Holding) Cooked rice (47F - Cold Holding). 11-03-5    Intermediate - Certified Food Manager or person in charge lacks knowledge of employee health policy and health requirements. Provided poster to operator. **Warning**
11-27-4    Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Provided operator with poster . **Warning**
03D-15-4    Intermediate - Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Front reach in cooler - cut lettuce at 10:00am- (60F - Cooling). Per operator item was prepared this morning at approximately 7am. At 11:00 am cut lettuce -(60F-Cooling. Educated operator on proper cooling methods. Operator moved to freezer for quick chill. **Warning**
