A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust.
Observed accumulation of food debris around chest freezer door rim. **Warning**
Basic - Opened employee beverage container in a cold holding unit with food to be served to customers.
Observed employee drink stored inside reach in freezer in kitchen. Employee removed. **Corrected On-Site** **Warning**
High Priority - Roach activity present as evidenced by live roaches found.
1) Observed approximately 3 live roaches, in kitchen, on wall behind reach in cooler next to back kitchen exit door.
2) Observed approximately 2 live roaches in between wall and baseboard behind clean dish ware rack, in kitchen.
3) Observed 1 live roach on clean dish ware stored on dish ware rack, in kitchen.
4) Observed 1 live roach on floor in front of flip top cooler, in kitchen.
5) observed 1 live roach on wall, above triple sink. **Warning**
Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment.
Provided vomit and diarrhea cleanup procedures to operator. **Warning**
Intermediate - Operator is not properly tracking/marking the number of days ready-to-eat, time/temperature control for safety food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again.
Observed cooked pork, prepared onsite, stored inside freezer without date marking. Per operator, food prepared approximately 6 days ago. Operator put date mark on food. **Corrected On-Site** **Warning**
Intermediate - Spray bottle containing toxic substance not labeled.
Observed spray bottles containing clear and green unknown solutions, stored at front counter sink. Per operator, bottles filled with bleach water and soap. Operator labeled. **Corrected On-Site** **Warning**
