Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

13-07-4    Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed staff doing food handling while wearing watch at oven area. **Warning**
14-11-5    Basic - Equipment in poor repair. Observed walk-in cooler and freezer gaskets in disrepair. Observed walk-in cooler, walk-in freezer, and reach-in cooler gaskets, across from fryers in disrepair **Warning**

10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed bowl with no handle used to scoop grated Parmesan cheese at front counter. **Warning**

05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. Observed no ambient thermometer inside reach-in cooler across from fryers. **Warning**
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Observed at hand washing sink across from large ovens. **Warning**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach-in coolers gaskets at front counter soiled. **Warning**

08B-12-5    Basic - Stored food not covered. Observed chicken wings, sliced beef, and fries not covered while stored at reach-in cooler at front counter. **Warning**
36-02-5    Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Observed at three compartment sink area. **Warning**
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed at shelves inside walk-in cooler. **Warning**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed plastic containers storing white powder on counter leading to bathrooms with no labels. **Warning**
03D-02-5    High Priority - Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Observed cooked shredded chicken (50F - Cooling). As per person in charge, prepared last night. **Warning**
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed chicken wings (50F - Cold Holding); young chicken wings/fillet (48F - Cold Holding): shredded cheese (45F - Cold Holding); heavy cream (45F - Cold Holding); cooked shredded chicken (50F - Cooling), inside walk-in cooler. Also observed, shredded cheese (47F - Cold Holding); shredded chicken (66F - Cold Holding) at reach-in cooler across fryers. As per person in charge, over 4 hours. **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed raw chicken wings (50F - Cold Holding); young chicken wings/fillet (48F - Cold Holding): shredded cheese (45F - Cold Holding); heavy cream (45F - Cold Holding) inside walk-in cooler. Also observed, shredded cheese (47F - Cold Holding); shredded chicken (66F - Cold Holding) at reach-in cooler across fryers. As per person in charge, over 4 hours. **Warning**
03B-01-6    High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed Cheese pizza (98F - Hot Holding); Pepperoni and cheese (100F) at front counter display. As per manager less than 1 hour. Pizza are being kept under time, with time tracked but there are no written procedures. **Warning**
16-37-1    Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. **Warning**
05-08-4    Intermediate - No probe thermometer provided to measure temperature of food products. **Warning**
53B-01-5    Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed that only the manager was working, but he said that he has certification for other staff but don't have them at the moment. **Warning**
03F-10-5    Intermediate - No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Observed pizzas under time with time being tracked, but no written procedures at front counter display case. **Warning**
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed two plastic spray bottles not labeled at front counter. **Warning**
