Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

06-09-1    Basic - Commercially processed reduced oxygen packaged raw tuna bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. **Repeat Violation**
14-11-5    Basic - Equipment in poor repair. Observed reach in cooler gaskets torn and warewash shelves in disrepair.
08B-38-4    Basic - Food stored on floor. Observed bucket of sauce and oil on floor in kitchen area.
14-36-5    Basic - Observed Interior of freezer in disrepair and has exposed insulation. **Repeat Violation**
08B-39-4    Basic - Observed avocado not washed prior to preparation.
29-49-6    Basic - Standing water in bottom of reach-in-cooler. Chef cleaned. **Corrected On-Site**
14-86-1    High Priority - Non-food grade paper/paper towel used as liner for food container. Observed non food grade paper towels lining various pans of food in reach in coolers.
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw salmon stored over various ready to eat foods in reach in cooler.
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink.
11-26-1    Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Discussed and emailed form to operator.
22-31-4    Intermediate - Non-pitting surface rust on food-contact equipment. Observed surface rust on prep tables and shelves in kitchen area.
