A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Ceiling tiles and vents soiled with accumulated food debris, grease, dust, or mold-like substance throughout kitchen and prep areas. **Warning**
Basic - Dead roaches on premises.
1. Observed 2 dead roaches on floor next to reach in cooler at cookline in kitchen.
Operator started to clean areas at the time of the inspection. **Warning**
Basic - Wall soiled with accumulated grease, food debris, and/or dust.
1. Observed on wall at dishmachine in kitchen.
2. Observed on wall behind cookline in kitchen. **Warning**
High Priority - Live, small flying insects found
1. Observed approximately 10 live, small flying insects resting on wall and on cleaning equipment at mop sink in kitchen.
2. Observed two live, small flying insects resting on shelving opposite cookline in kitchen.
Operator started to clean areas of the kitchen at the time of the inspection. **Warning**
High Priority - Presence of insects, rodents, or other pests.
1. Observed approximately 12 live insects crawling on packaged dry pasta on shelf in dry storage area.
2. Observed approximately 20 live insects inside boxes of Barilla pasta. See Stop Sale. **Warning**
High Priority - Roach activity present as evidenced by live roaches found.
1. Observed 7 live roaches crawling on floor at cookline in kitchen.
2. Observed 4 live roaches in oven at cookline in kitchen.
3. Observed one live roach crawling on two door reach in cooler at omelet station.
Operator killed roaches, started to clean areas of at the time of the inspection.

High Priority - Stop Sale issued due to food not being in a wholesome, sound condition.
Observed approximately 20 live insects inside two boxes of Barilla pasta. **Warning**
