that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.

High Priority - Heat strip failed to turn black to indicate the sanitization temperature of 160 degrees Fahrenheit was achieved on the dish surface and/or high temperature holding thermometer did not reach 160F. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired.

High Priority - Live, small flying insects found
In kitchen approximately 1 live flying insect flying around flip top cooler.
In kitchen approximately 5 live flying insects landing on dish machine and wall next to mop sink.

High Priority - Sewage/wastewater backing up through floor drains.
In kitchen waste water backing up from floor drain in front of hand washing sink and vegetable wash sink, area cannot be isolated and employees stepping in grey water, tested all sinks and water facilities, discussed with operator to repair plumbing. **Warning**
High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit.
In steam table house made chili (121F - Hot Holding) per operator product prepared approximately 30 minutes ago on stove and moved to hot holding unit, discussed with operator who turned on steam table and moved chili to flat top and reheated to 165F+. **Corrected On-Site** **Warning**
