A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance.

Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package.

Basic - Cove molding at floor/wall juncture broken/missing.
Throughout kitchen multiple places cove molding missing/ broken, discussed with operator to repair or replace. **Warning**
Basic - Dead roaches on premises.
In kitchen approximately 1 dead roach shelf above food preparation sink.
In kitchen approximately 1 dead roach on floor in dry storage room.
In kitchen approximately 7 dead roaches on floor by grease collector in between walk in cooler door and walk in beer cooler, discussed with operator to clean and sanitize areas.

Basic - Employee eating while preparing food.
Employee In kitchen eating pasta while preparing bacon, discussed with operator who educated employee. **Corrected On-Site** **Warning**
Basic - Floors not constructed to be easily cleanable.
In kitchen in at cook line and server expo station floor tile grout missing with food and liquid debris in between tile, discussed with operator to repair, clean and sanitize. **Warning**
High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands.
Employee eating in kitchen did not wash hands before handling containers of bacon, discussed with operator who educated employee, employee washed hands. **Corrected On-Site** **Warning**
High Priority - Live, small flying insects found
In kitchen approximately 30 live flying insects landing on mop and mop bucket next at entrance to walk in cooler.
In kitchen approximately 10 live flying insects landing on trash can and soda dispenser rack in front of walk in cooler.
In kitchen approximately 15 live flying insects flying in and around floor drain at server expo station under soda machine.
Discussed with operator to kill all flying insects and clean and sanitize areas. **Warning**
High Priority - Roach activity present as evidenced by live roaches found.
In kitchen approximately 1 live roach crawling on shelf and sealed box above food prep sink where employee activity preparing food, discussed with operator who killed roach and began to clean and sanitize area.
In kitchen approximately 1 live roach crawling on floor behind ice machine, operator killed roach.
In kitchen approximately 1 live roach behind oven on floor.
Discussed with operator to eliminate all roaches and clean and sanitize areas. **Warning**
High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse.
