Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

16-03-4    Basic - Accumulation of debris inside warewashing machine.
16-21-4    Basic - Accumulation of debris on exterior of warewashing machine.
36-36-4    Basic - Ceiling tile missing. Observed ceiling tiles missing in the dining room area.
24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed at storage shelf above prep cooler in kitchen, bowls and plates stored not inverted for protection. During the inspection employee removed items and placed them correctly. **Corrected On-Site**
06-09-1    Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed tuna in individual vacuum packages inside reach in cooler, located at cooking line. The operator opened the packages and removed the tuna from its original packaging. **Corrective Action Taken**
35A-03-4    Basic - Dead roaches on premises. Observed approximately 17 dead roaches inside the out-of-service ice machine located in the preparation kitchen.
14-11-5    Basic - Equipment in poor repair. Observed one of the ice machines located in the preparation kitchen was observed to be non-operational.
36-24-5    Basic - Hole in or other damage to wall. Observed hole in the wall underneath the three compartment sink.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed hood system soiled. Observed most of reach in cooler gaskets soiled.
29-49-6    Basic - Standing water in bottom of reach-in-cooler, located at cooking line.
33-34-4    Basic - Storage area not maintained clean and organized. Observed unused kitchen Equipments ( oven, ice machine, etc ) on the back kitchen.
14-17-4    Basic - Walk-in cooler shelves with rust that has pitted the surface.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses, throughout the cooking line area.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed bottled with oils in the kitchen area not properly label.
12A-07-5    High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed an employee drying their hands with a soiled, wet cloth and then proceeded to put on gloves without proper hand washing. The employee was coached on proper hand wash procedures. Following the guidance, the employee washed their hands and then properly donned gloves. **Corrective Action Taken**
35A-02-7    High Priority - Live, small flying insects found. Observed approximately 13 or more small flies flying in the bar area near the ice bin and hand sink. Observed approximately 5 flies seen landing on top of drinking straws.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed Two roaches crawling on top of the preparation table in the preparation kitchen. Additionally, one roach was seen on the wall adjacent to the mop sink in the preparation kitchen, and two more were observed on the wall near the hot holding unit along the cooking line.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 18 rodent droppings underneath the three-compartment sink, located at preparation kitchen.
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bib, located underneath the hand sink located at preparation area.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed can opener blade soiled.
53B-05-5    Intermediate - Required employee training expired for all employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed all employees food handlers training expired, Jean Duvain, Andrei Cosmin , Relu Turlio, and Luis Ruiz.
