The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.

Observed one employee metal container with dressing without handle in ice machine. **Warning**

Observed one employee plastic container with dressing on floor in walk-in refrigerator.

Basic - Label on a food item prepared and packaged onsite for customer self-service does not include product identity/description, date product was packaged and name and address of the establishment that prepared and packaged the food.
Observed at front counter baklava. **Warning**
Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust.
Observed can grease on hood vent and floor underneath stoves and refrigerator. **Warning**
Basic - Working containers of food removed from original container not identified by common name. Observed oil and salad dressing and squeeze bottles not labeled.. **Repeat Violation** **Admin Complaint**
High Priority - Certified Food Manager or person in charge failed to verify employee health, exclusions or restrictions.
Certified food manager does not know probably restrict an employee from coming to work permanent diarrhea flu symptoms. **Warning**
High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature.
Observed raw chicken above raw salmon and beef in walk-in refrigerator..
Operator moved salmon and beef and placed chicken on lower shelf. **Corrected On-Site** **Warning**
High Priority - Roach activity present as evidenced by live roaches found.
Observed approximately 3 live roaches in can opener in kitchen at cook line, one live roach by soda bottle storage in kitchen. **Warning**
Intermediate - Employee has not received adequate training related to their assigned duties as evidenced by lack of general knowledge in performing their assigned duties.
Observed employee that is responsible of ware washing
That doesn't know how to set up 3 compartment sink.
He washed then rinsed and then mix chlorine with soap and soaked, and then rinsed off. **Warning**
Intermediate - Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection.
Observed chicken at 12:12 with 2 hours to go. (125F - Cooling) at 12:34 (120F).
Due to chicken being in deep hotel pan at room temperature.
Operator placed in multiple shallow hotel pans, and place the refrigerator uncovered. **Corrective Action Taken** **Warning**
