Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

24-06-4    Basic - Clean utensils or equipment stored in dirty drawer or rack. Observed a knife and two knife sharpers stored on a dirty shelf of ice machine. **Warning**
24-07-4    Basic - Cleaned and sanitized equipment or utensils not properly stored. Observed clean cups not protected from splashing being stored right next to hand wash sink. **Warning**
14-74-7    Basic - Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed food inside reach in cooler were out of temperature and thermometer was reading at 34°F . No ambient thermometer inside the reach in cooler. **Warning**
32-12-6    Basic - Covered waste receptacle not provided in women's bathroom. **Warning**
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Observed the cutting board of prep table with deep grooves. **Warning**
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed personal bag stored on prep table. **Warning**
13-07-4    Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee wearing bracelet while preparing food. **Warning**

24-08-4    Basic - Equipment and utensils not properly air-dried - wet nesting. Observed ceramic bowls and plates stored wet nesting on shelves in dry storage area. Also observed plastic food containers wet nesting near mop sink. **Warning**
14-11-5    Basic - Equipment in poor repair. Observed the inside panel door of walk in freezer in disrepair, the door gaskets of reach in cooler, and handle of reach in cooler. **Warning**

08B-12-5    Basic - Stored food not covered. Observed an open can of evaporated milk and milk inside a metal container not covered in the reach in cooler next to coffee station. And two bowl of creme brulee inside the walk in cooler. **Repeat Violation** **Warning**
08B-17-4    Basic - Unwashed fruits/vegetables stored with ready-to-eat food. Observed a red onion with its skin on stored on top of ready to eat cut kale inside the baine marine of prep trolley. **Warning**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed mold like substance on the walls around mop sink and dishwashing machine area. **Warning**
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Observed leaking at the pipe of hand wash sink next to walk in cooler. **Warning**
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. Observed two wet mops stored on the floor not allowing to dry near dishwasher. **Repeat Violation** **Warning**
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed a wet wiping cloth stored on a prep table. **Warning**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed no label on the plastic container containing sugar near coffee station. **Warning**
35A-02-7    High Priority - Observed approximately 20 live, small flying insects found around dry storage area, mop sink and at the dishwasher area. **Admin Complaint**
41-05-4    High Priority - Pesticide/insecticide labeled for household use only present in establishment. Observed two cans of RAID spray stored on a shelf above prep tables. **Warning**
08A-04-5    High Priority - Raw animal food stored over or with unwashed produce. Observed raw chilled eggs stored next to unwashed potatoes. Manager separated the potatoes to another shelf. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed one rodent dropping on top of syrups in dry storage area. **Admin Complaint**
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed cut tomatoes (50F - Cold Holding); feta (50F - Cold Holding); cut kale (48F); raw shrimp (47F - Cold Holding), in the reach cooler. Stop sale was issued. **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cut tomatoes (50F - Cold Holding); feta (50F - Cold Holding); cut kale (48F); raw shrimp (47F - Cold Holding), in the reach cooler. Stop sale was issued. **Warning**
41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed stored in the floor under hand wash sink that's in between preparing tables. Also stored on prep table next to soda machine and also stored above clean containers by the mop sink. **Warning**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no vacuum breaker at the mop sink bib and the bib connecting to a hose next to hand wash sink. **Warning**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed the orange juice press, can opener, inside ice machine and soda machine nozzles soiled. **Repeat Violation** **Warning**

14-14-4    Intermediate - Nonfood-grade basting brush used in food. Observed non food grade brush with metal used at the coffee station. **Warning**
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed no date marking for potato salad, cut tomatoes, cut kale, creme brulee and cooked yuca inside the walk in cooler. Also observed an open can and milk in a metal container in the reach cooler next to coffee station. Per operator both milks were not opened today. **Warning**
