Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-73-4    Basic - Floor soiled/has accumulation of debris. Observed floor soiled throughout kitchen floor, behind cook line.
36-24-5    Basic - Hole in or other damage to wall. Observed wall in disrepair behind dishwasher. **Repeat Violation**
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Observed at pizza station.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler gaskets soiled. Observed hoods soiled with heavy grease accumulation.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed wall with accumulation of grease at cook line. **Repeat Violation**
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed throughout kitchen.

35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 3 live roaches crawling on the floor by the food storage area. Observed 2 live roaches crawling on the floor at dishwasher area room.
03A-03-5    High Priority - Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. Observed raw shell eggs stored at ambient temperature at cook line. As per operator for less than 1 hour prior, coached him on correct procedures.
02B-01-5    Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Observed egg Benedict, huevos rancheros no linked to the consumer advisory. **Repeat Violation**
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed white rice, cooked pasta, cooked beef inside reach in cooler no date marked, as per chef for more than 24 hours prior.
53B-05-5    Intermediate - Required employee training expired for all employees.
