Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed in walk in cooler container of grouper ceviche, not for establishment stored with establishments food items. Operator removed and discarded. **Corrected On-Site**
10-08-5    Basic - Ice scoop handle in contact with ice. Observed at pizza area ice bin. **Repeat Violation**
10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed at cook line in water of 81F. **Repeat Violation**
16-38-5    Basic - Inaccurate/damaged temperature and/or pressure gauge(s) on dishmachine. Observed gauge stuck at 150F.
08B-12-5    Basic - Stored food not covered. Observed at walk in cooler, tray of raw chicken uncovered. **Repeat Violation**
36-02-5    Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Observed at entrance and storage area of kitchen area. **Repeat Violation**
50-17-3    High Priority - Operating with an expired Division of Hotels and Restaurants license. Observed expired dated 10/1/25.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed at walk in cooler tuna salad (53F - Cold Holding) as per label from 11/3 and fish broth (51F - Cold Holding) as per label from 11/11. Operator removed and discarded. **Corrected On-Site** **Repeat Violation**
03B-01-6    High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed at cook line a container of melted butter (83F - Cold Holding). As per chef for less than 2 hours. Chef began to increase heat to reheat. **Corrective Action Taken** **Repeat Violation**
