Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

06-09-1    Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed thawed tuna inside ROP bag at walk in cooler.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee food items stored next to establishment food inside walk in freezer at fish preparation area.
14-11-5    Basic - Equipment in poor repair. Observed gaskets torn inside reach in cooler at cook line area.
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Observed on back door at preparation area.
36-22-4    Basic - Floor area(s) covered with standing water. Observed all around kitchen area.
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed walk in freezer floor soiled with food debris. Employee clean floor during inspection. **Corrected On-Site**
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed next to the wall at walk in freezer. Also observed inside chest freezer at cook line area.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets soiled inside reach in cooler outside of walk in cooler. Operator cleaned it during inspection. **Corrected On-Site**
14-17-4    Basic - Shelves with rust that has pitted the surface. Observed shelf rusty next to dishwasher machine at kitchen area.
29-49-6    Basic - Standing water in bottom of reach-in-cooler. Observed standing water inside reach in cooler outside of walk in cooler. Also observed standing water inside reach in cooler at cook line area.
36-02-5    Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Observed at preparation area at kitchen area.
14-12-4    Basic - Utensils in poor condition. Observed 3 clean plastic containers broken on shelf next to dishwasher machine at kitchen area.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed panko bread container with not labeled on it. Operator labeled it during inspection. **Corrected On-Site**
02C-01-5    High Priority - Ready-to-eat, time/temperature control for safety food marked with a date that exceeds 7 days after opening/preparation. Observed mango slow salad dated 12/09 on shelf at walk in cooler. Employee discarded during inspection. **Corrected On-Site**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed thawed tuna inside ROP bag at walk in cooler.
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Also observed Oyster (46 F - Cold Holding) inside cold holding unit at front counter. As per operator for more than 4 hours. Operator discarded during inspection. **Corrected On-Site**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Egg Wash (51F - Cold Holding) at cook line. Operator placed with ice to cool it down properly. Also observed Crab (51F - Cold Holding); Crab (46F - Cold Holding) inside cold holding unit at front counter. As per operator for less than 4 hours. Operator removed it and stored inside walk in cooler to cool it down properly. Also observed Oyster (46 F - Cold Holding) inside cold holding unit at front counter. As per operator for more than 4 hours. Operator discarded during inspection. **Corrective Action Taken**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed next to reach in cooler outside of walk in cooler at kitchen area.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed can opener blade soiled at preparation table at kitchen next to walk in cooler.
