Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Observed cutting board grooved by three compartment sink.
36-24-5    Basic - Hole in or other damage to wall. Observed wall damage by salad station. Observed wall damage by employee bathroom upstairs.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler gaskets soiled by pizza station and salad station. Observed walk in cooler fan soiled.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed wall by three compartment sink dusty.
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Observed water leaking from pipe under hand sink by three compartment sink.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed sugar container not labeled across oven by storage.
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Observed pizza tray stored on top by hand sink. Employee removed. **Corrected On-Site**
