Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Observed male cook line employee engaged in food preparation with no hair restraint. **Warning**
24-08-4    Basic - Equipment and utensils not properly air-dried - wet nesting. Observed aluminum containers at the dry rack station with water accumulation on the rim. **Warning**

36-50-4    Basic - Unclean building components, attachments or fixtures. Observed building components behind the cook line soiled. **Warning**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed wall near mop sink soiled. Observed wall behind main cook line soiled. **Warning**
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloth on top of white cutting board located across the cook line not stored in sanitizing solution. **Warning**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed several working containers of seasonings and oils not labeled throughout the prep area and cook line. **Warning**
41-05-4    High Priority - Pesticide/insecticide labeled for household use only present in establishment. Observed house hold pesticide spray underneath prep table with food seasonings next to it. **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed one live roach above hand wash sink located at the sushi bar area. **Warning**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Observed sanitation bucket inside hand wash sink at the sushi bar area. **Warning**
