Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

22-20-5    Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin that is used for drinks. Employee began to clean ice machine. **Corrective Action Taken** **Warning**
06-09-1    Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. In the stand alone cooler across from the handwashing sink in main kitchen, observed four packs of fully thawed raw salmon in commercially reduced oxygen packaging. **Warning**
16-46-4    Basic - Old labels stuck to food containers after cleaning. In the stand alone cooler next to the ice machine, observed a container of cooked vegetables dated 4/7. Operator stated vegetables were made the day prior to inspection. Operator stated the label was an old label. **Warning**
12A-20-4    High Priority - Employee washed hands with no soap. Discussed with operator and employee the proper way to wash hands. Employee began to wash hands with soap. **Corrective Action Taken** **Warning**
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. In the stand alone reach in cooler across from the handwashing sink observed Raw shrimp stored above ready to eat chicken. Raw sausage stored above ready to eat cooked chicken. Operator rearranged for proper storage. **Corrected On-Site** **Warning**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. 12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. Observed food handler employee wash hands in the three compartment sink. Discussed with operator handwashing should be done at only handwashing sinks. **Warning**
31A-04-4    Intermediate - Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed. Per approved plans hand sink required next to the grill. Operator stated hand sink was removed. **Warning**
