Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

06-09-1    Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Five pieces of tilapia at 34F. Employee began removing fish from package. **Corrective Action Taken**
14-11-5    Basic - Equipment in poor repair. Two reach in coolers have broken handles. Wood blocks under microwave not sealed. Cutting board has deep grooves. **Repeat Violation**
10-18-5    Basic - In-use utensil stored in sanitizer between uses and/or between equipment. Three knives in sanitizer at end of cook line. Three knives between wall and make table. One knife between make table and prep table. Employee removed. **Corrected On-Site** **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Hood filters soiled with dust and grease. Fan in ware wash area soiled with dust. Around doors of reach in salad cooler soiled with black substance.
24-17-4    Basic - Silverware/utensils/equipment dried with a towel/cloth. Female employee dried plates with towel then placed with clean dishes.
12A-13-4    High Priority - Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Female employee placed dirty dishes in ware wash area then picked up Togo tray without washing hands. **Repeat Violation** **Admin Complaint**
