Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

08B-49-4    Basic - Employee personal food not properly identified and segregated from food to be served to the public. In reach in cooler near your front door, employee fruit on shelf over drinks . In reach in cooler in server station, strawberries and cream on shelf over condiments. In reach in cooler in back room, cabbage on top of pan of sauce.
14-38-4    Basic - Food storage container/container lid cracked or broken. Bin containing dough in reach in cooler broken. Deflector plate in ice machine broken.
08B-38-4    Basic - Food stored on floor. Bin of chips on floor on cook line.
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Large chunk of ice on bag chicken in reach in freezer and around door of walk in freezer. **Repeat Violation**
10-17-4    Basic - In-use knife/knives stored in cracks between pieces of equipment. Two knives between prep tables and make table on cookline.
10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Scoop handle inside beans in back room. Removed during this inspection. **Corrected On-Site**
38-01-4    Basic - Light shield damaged/in disrepair. Light shield hanging from ceiling in prep area
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Top of dish machine soiled with food debris and dust. Fan in prep area heavily soiled with dust.
35B-05-4    Basic - Outer openings not protected during operation and vermin and/or environmental cross contamination present. Several holes around door trim at back door near ice machine. **Warning**
08B-39-4    Basic - Raw fruits/vegetables not washed prior to preparation. Avocados in pan with stickers still attached in make table on cook line. **Repeat Violation**
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. White trays on cookline not inverted.
21-10-4    Basic - Soiled dry wiping cloth in use. Two towels on cookline heavily soiled with food debris.
29-49-6    Basic - Standing water in bottom of reach-in-cooler. In bottom of cooler in server station.
08B-12-5    Basic - Stored food not covered. Pans of pork, chicken and chilis not covered in walk in freezer. **Repeat Violation**
08B-17-4    Basic - Unwashed fruits/vegetables stored with ready-to-eat food. Unwashed produce on shelf over cut onions in back room reach in cooler.
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Water dripping from hand wash sink on cookline.
12A-29-4    High Priority - Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Male employee touched soiled towel then stirred queso without washing his hands.
41-05-4    High Priority - Pesticide/insecticide labeled for household use only present in establishment. Can of raid household spray on shelf next to dish machine.
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. In reach in cooler located in back storage room, pan of raw ground beef over container of sour cream. In reach in freezer in back room, raw pork in bins stored over mango and fries. Also, in reach in cooler on cook line, pan of raw chicken on shelf over steak Also, employee preparing raw meats on prep table over sliced onions, and bag of onions. **Repeat Violation** **Admin Complaint**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Approximately 20 rodent droppings in air handler closet near server station. Seven rodent droppings on floor under cooking equipment in kitchen area. Ten rodent droppings on floor under reach in cooler, dish machine, and prep table in prep room. **Repeat Violation** **Admin Complaint**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Bin of chicken not covered in walk in cooler with large ice chunks through out. Stop sale issued.
41-10-4    High Priority - Toxic substance/chemical improperly stored. Bottle of hand sanitizer on top of paper towel dispenser in prep room.
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Splitter added at mop sink. One side with hose not attached missing device. **Repeat Violation** **Admin Complaint**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Interior of reaching cooler on cook line soiled with food debris. Interior of reach in freezer in back room soiled with food debris. Interior microwave on cook line soiled with food debris. Interior of microwave in prep area, soiled food debris.
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Mop bucket and mop in front of hand sink near dry storage. This was moved during this inspection. **Corrected On-Site**
53B-10-4    Intermediate - Records/documents for required employee training do not contain all of the required information. One certificate provided missing certificate number of trainer
