Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

32-05-4    Basic - Bathroom facility in disrepair. Observed men's bathroom with an out of service sign. As per manager the toilet is overflowing and the plumber is on the way to fix the issue.
14-11-5    Basic - Equipment in poor repair. Observed reach in cooler near the walk in cooler used as dry storage.
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed floor under cooking equipments soiled at cook line.
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup at reach in freezer near office computer.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gasket at sauce reach in cooler soiled located at kitchen entrance.
29-02-4    Basic - Toilet leaking. Observed toilet overflowing. As per manager the plumber is on the way to fix the issue.
12A-16-4    High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed manager and employee started preparing and packing food like chicken, stir fry noodles to serve to customers. No handwash. Coach employee.
12A-07-5    High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee and manager put on gloves and started preparing and plating food like grilled chicken, rice to serve to customers. No handwash. Coach manager.
22-43-4    High Priority - Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed Triple Sink (Quaternary 0ppm). Manager reset triple sink. Final reading Triple Sink (Quaternary 200ppm). **Corrected On-Site**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed shelf with clean pans under stove soiled.
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Observed handwash sink near the triple sink blocked with a black container of degreaser. Coach manager.
22-31-4    Intermediate - Non-pitting surface rust on food-contact equipment. Observed clean dishes shelves above the triple sink rusted.
