A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Dead roaches on premises. Observed 1 dead roach under steamer on cook line in kitchen area. Observed 3 dead roaches inside of exposed insulation of ice machine. Observed 1 dead roach inside of plastic to go lids in kitchen area. Observed 1 dead roach under freezer across mop sink in kitchen area. Observed 1 dead roach in kitchen area that fell out of wet mop. Observed 2 dead roaches under triple sink in kitchen area. Observed 1 dead roach in floor drain under triple sink in kitchen area. **Repeat Violation** **Warning**
Basic - Equipment and utensils not properly air-dried - wet nesting. Observed pots and pans under cook line wet nesting. Observed salad spinner wet nesting on dry food rack in kitchen area. **Warning**
Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed whisk and spoon in standing water on dish rack next to cook line. **Warning**
Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets soiled in reach in coolers in kitchen area. **Warning**
Basic - Objectionable odors in bathroom or other areas of the establishment. Observed in kitchen area under triple sink and at floor drain. **Repeat Violation** **Warning**
High Priority - Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. Observed employee washing pans with soap and water; no sanitizer used. **Warning**
High Priority - Raw animal food stored over or with unwashed produce. Observed raw salmon and tuna over case of avocados in reach in cooler next to cook line. **Warning**
High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw salmon and masago stored above seaweed salad in reach in next to cook line. **Repeat Violation** **Warning**
High Priority - Roach activity present as evidenced by live roaches found. Observed 1 live roach under triple sink in kitchen area. **Repeat Violation** **Warning**
