A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed in kitchen and front counter. **Warning**
Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed can of coke on top of reach in cooler on cook line. Employee tossed out soda. **Corrected On-Site** **Repeat Violation** **Warning**
Basic - Interior of refrigerator or freezer in disrepair/has exposed insulation. Observed at ice machine in kitchen area. **Repeat Violation** **Warning**
High Priority - Live, small flying insects found. Observed 3 live flies on wall next to cook line in kitchen area. Observed 3 live flies on dry food and dish rack next to reach in cooler on cook line in kitchen area. Observed 2 live flies on wall in dining area. Observed 3 live flies above front counter where miso soup and utensils and plates are held, near air vent. Observed 8 live flies on wall and extension cord at kitchen area entry. **Warning**
High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw salmon stored above edamame in cooler next to cook line. Operator changed order during inspection. **Corrected On-Site** **Warning**
