A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-73-4    Basic - Floor soiled/has accumulation of debris. Observed walk in cooler floor soiled. Observed floor under reach in cooler front of walk in cooler soiled and under freezer. **Warning**
36-24-5    Basic - Hole in or other damage to wall. Observed wall in disrepair at back door entrance. **Warning**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler gaskets soiled front counter by smoothie maker. **Repeat Violation** **Warning**

22-49-4    High Priority - Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed Dishwasher (Chlorine 0ppm). **Warning**
12A-07-5    High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee changing task. Discarded gloves and put new gloves without wash hands. Coached employee to wash hands properly. **Warning**
03B-01-6    High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed front counter; white rice (112F - Hot Holding); cooked chicken (98F - Hot Holding); brown rice (123F - Hot Holding) at steamer. As per manager less than 2 hours.
