Violations:
A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices.
Observed approximately 100+ dead flies in the bug zapper on top of reach in freezer. **Repeat Violation** **Warning**

Observed approximately 10+ live flies in the back storage room flying around and landing on top of clean juice containers and clean blenders.
Observed 3+ live flies inside the walk in cooler flying around.
Observed 4 live flies laying on top of a container with jerk chicken inside.
Observed 5+ live flies on top of raw chicken lids where juices are being stored in. **Repeat Violation** **Warning**
High Priority - Rodent activity present as evidenced by rodent droppings found.
Observed approximately 3 droppings on the floor by the back storage shelves. **Repeat Violation** **Warning**
Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. **Repeat Violation** **Admin Complaint**

Observed the approved plans do not match the current layout of the establishment. Observed establishment added kitchen area behind the bar with a stove hood system, reaching cooler, prep tables and sink, walk-in cooler walk-in freezer and meat slicer. observed four big smokers behind the establishment, cooking jerk chicken. **Repeat Violation** **Admin Complaint**
