Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-01-4    Basic - Floor not cleaned when the least amount of food is exposed. Observed banana on floor around reach in coolers throughout kitchen.

21-07-4    Basic - Wiping cloth chlorine sanitizing solution not at proper minimum strength. Observed below preparation table below blender station Sanitizer Bucket (Quaternary 0ppm). Operator corrected concentration of Sanitizer Bucket (Quaternary 200ppm). **Corrected On-Site**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed on cart to the left of smoothie section no label on container containing chocolate powder, whey, sugar.
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed in reach in cooler to the right of microwave 11 portions of kale salad (53F - Cold Holding) as per operator stored longer than 4 hour. Operator discarded kale salad. **Corrected On-Site**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed in reach in cooler across from preparation table cut tomato (51F - Cold Holding), cooked chicken (50F - Cold Holding), spinach (51F - Cold Holding), sliced turkey (51F - Cold Holding), as per operator less than 4 hours. Operator stored on ice for rapid cooling. **Corrective Action Taken**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed at smoothie station soiled with food debris blenders and blender lids.
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Observed hand wash sink to the right of ice machine blocked by preparation table. Operator moved table during time of inspection. **Corrected On-Site**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towel in hand wash sink to the right of ware wash area. Operator provided paper towel during time of inspection. **Corrected On-Site** **Repeat Violation**
53B-14-5    Intermediate - Required employee training expired for some employees.
