Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee food stored inside reach in cooler next to food for service.
36-17-5    Basic - Floor tiles missing and/or in disrepair. Observed tiles cracked and missing from kitchen floor. **Repeat Violation**
22-08-4    Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed interior of microwaves in kitchen soiled. **Repeat Violation**
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Observed no handwashing sign at handwashing sink near salad station.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed hood filters soiled. **Repeat Violation**
33-16-4    Basic - Open dumpster lid.
35B-03-4    Basic - Outer openings not protected with self-closing doors. Observed exterior bathroom doors without self closing doors.
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Observed to go containers not inverted on expo line.
08B-12-5    Basic - Stored food not covered. Observed food uncovered inside of walk in cooler. **Repeat Violation**
08B-60-4    Basic - Unpackaged food not protected from environmental sources of contamination during preparation. Observed open package of frozen hot dogs stored on rusted shelf in walk in freezer. Observed rust on unpackaged hotdogs. Operator discarded hot dogs. **Corrective Action Taken**
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed walk in freezer shelves rusted.
12A-07-5    High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee crack raw shell eggs, removed gloves and applied new gloves without handwashing. Employee discarded new gloves and washed hands. **Corrective Action Taken**
14-31-5    High Priority - Nonfood-grade bags used in direct contact with food. Observed bread rolls stored inside black garbage bag inside of walk in cooler.
35A-09-4    High Priority - Presence of insects and other pests. Observed lizard droppings on exterior of establishment on window seal. No observation or any pests or pest droppings inside establishment.
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw pork stored above ready to eat Sausage in walk in cooler. Operator corrected storage. **Corrected On-Site**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Observed open package of frozen hot dogs stored on rusted shelf in walk in freezer. Observed rust on unpackaged hotdogs. Operator discarded hot dogs.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed the following food stored in kitchen on cook line measuring above 41F Hashbrowns (67F - Cold Holding); Sliced Cheese (52F - Cold Holding).Sliced cheese and hashbrowns were stored on cook line 1 hour prior to inspection per operator. Observed Waffle Mix stored at room temp measuring (84F - Cold Holding). Waffle mix stored at room temp for 30 mins per operator. All items placed inside cold holding units. Observed the following items above 41F Ham (47F - Cold Holding); Coleslaw Dressing (44F - Cold Holding); Chicken Soup (47F - Cold Holding). Operator stated they had received food delivery prior to inspection.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed slicer soiled on prep counter.
14-14-4    Intermediate - Nonfood-grade basting brush used in food. Observed paint brush with metal binding stored inside melted butter on hot well.
