Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. In general, critical violations are those that, if not corrected, are more likely to contribute directly to food contamination, illness or environmental damage. Although we use the industry-standard term "critical", varying degrees of severity and potential risk to the public require inspectors to assess each situation in determining the appropriate action. In addition, while an establishment may have multiple violations, the inspectors' training and judgment formulate the overall result of the inspection to ensure the public health and safety. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

04-01-1    Critical - Cold holding equipment incapable of maintaining potentially hazardous food at proper temperatures. Pizza prep unit. Do not store any potentially hazardous foods in this unit until it is able to maintain food at 41 degrees F. or less.
31-09-1    Critical - Handwash sink not accessible for employee use at all times, pizza preparation area. Blocked by metal table.
32-17-1    Critical - Handwashing cleanser lacking at handwashing lavatory, bar.
27-16-1    Critical - Hot water not provided/shut off at mop sink.
10-04-1    In-use utensil for nonpotentially hazardous food not stored in a clean, protected location. 8 pr. Tongs on oven handle.
05-09-1    Critical - No conspicuously located thermometer in holding unit, by pizza oven.
32-15-1    Critical - No handwashing sign provided at handsinks used by food employees throughout establishment , including restrooms.
15-35-1    Nonfood-contact equipment exposed to splash/spillage not constructed of materials that are corrosion resistant, non-absorbent, smooth and easily cleanable. Bare wood shelf at dishwashing sink.
12B-07-1    Critical - Observed an open beverage container on a food preparation table or over/next to clean equipment/utensils, in bar.
13-02-1    Observed employee with ineffective hair restraint. Very long ponytail.

03A-07-1    Critical - Observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit. All potentially hazardous foods on top of pizza table.
02-07-1    Critical - Observed processed ready-to-eat, potentially hazardous food held more than 24 hours not properly date marked after opening. Cheesecake , tiramisu.
25-04-1    Observed single-service items stored on floor in back room.
41A-10-1    Critical - Observed toxic item improperly stored, over chest freezer in rear of establishment.
41B-03-1    Critical - Observed unlabeled spray bottle. On handsink, drink station.
35B-04-1    Critical - Outer openings of establishment cannot be properly sealed when not in operation. Back kitchen exit door rusted out at bottom. Can see daylight under door.
45-37-2    Critical - Portable fire extinguisher missing from designated location. For reporting purposes only. Front door, hook empty.
01B-16-1    Critical - Stop Sale issued on potentially hazardous food due to temperature abuse (see Stop Sale Notice).
21-12-1    Wet wiping cloth not stored in sanitizing solution between uses, pizza prep and cook area.
21-11-1    Wiping cloth chlorine sanitizing solution not at proper minimum strength- Drink station 0 ppm.
