A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
In general, critical violations are those that, if not corrected, are more likely
to contribute directly to food contamination, illness or environmental damage.
Although we use the industry-standard term "critical", varying degrees of severity
and potential risk to the public require inspectors to assess each situation in
determining the appropriate action. In addition, while an establishment may have
multiple violations, the inspectors' training and judgment formulate the overall
result of the inspection to ensure the public health and safety. While most
establishments correct all violations in a timely manner (often during the inspection),
the division's procedures are designed to compel compliance with all violations through
follow-up visits, administration action or closure when necessary.
Clean glasses, cups, utensils, pots and pans not stored inverted or in a protected manner. observed front line waiting station cleaned flatware storage in an inverted manner. Corrected On Site.
Critical - Observed a female employee waitress with artificial nails/nail polish working with exposed food who did not wash hands prior to doning gloves then handle ready to eat toast and expedite additional ready to eat foods.
Observed clean linens stored in improper location. observed 1 cleaned linen clear plastic storage bag stored on top of food meat boxes in walk in freezer. Items must be stored in a clean dry location . Corrected On Site.
Critical - Observed dead roaches on premises. observed one dead roach on kitchen dry storage shelf adjacent to single service items, observed 4 dead roaches on floor under kitchen dishmachine, observed 2 dead roaches under kitchen 3 compartment sink, observed 2 dead roaches on floor in dry storage room, observed 1 dead roach on floor in employee restroom , observed 2 dead roaches on floor under ice bin at front wait station,observed 2 dead roaches on shelf at slicer station.

Observed food debris accumulated on kitchen floor. observed excessive food debris underneath and between cookline equipment and along baseboards in kitchen. and under and between kitchen island

Critical - Observed food employee wearing jewelry other than a plain ring on their hands/arms while preparing food. observed a food preparation employee waitress serving ready to eat foods that are exposed to multiple finger rings and wrist bracelets.
Observed inside of dumpster not clean. observed dumpster interior excessively soiled with liquid garbage waste. observed the presence of several hundred maggots inside of dumpster .
Critical - Observed raw animal food stored over cooked food. observed Raw chicken stored over cooked potatoes in kitchen walk in cooler. Corrected On Site.
Critical - Observed roach activity as evidenced by live roaches found. observed one live roach on floor under kitchen dishmachne , observed 2 live roaches on floor at front wait station.

Critical - Observed the presence of insects, rodents, or other pests. observed exterior garbage dumpster interior excessively soiled with several hundred live maggots.
Critical - Observed two open beverage containers on a kitchen food preparation table at kitchen center island. Corrected On Site. drinks discarded by manager.
Observed walls soiled with accumulated dust. observed rear dry storage room walls including above kitchen entrance door way, behind syrup station, and around wall mop storage frame excessively soiled with dust and foods debris . Corrected On Site.
Critical - Outer openings of establishment cannot be properly sealed when not in operation. observed rear kitchen exit door frame damaged and rear door exit not adequately and completely sealed when rear door is closed.
