Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

03C-44-4    High Priority - Cooked poultry not reaching a minimum internal temperature of 165 degrees Fahrenheit for 15 seconds. Observed poultry cooked to 160° and put into to go container. **Corrected On-Site** **Warning**
35A-03-4    Basic - High Priority - Dead roaches on premises. Observed 2 dead roaches in women's rest room. Observed numerous random roach body parts to include legs, thoraxes, abdomens and wings on food containers, food cans, shelving and floor throughout prep area and storage area. **Warning**
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. On dry storage shelf. **Warning**
12A-16-4    High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed employee come in from outside and begin touching equipment and making rice without washing hands. **Warning**
12A-20-4    High Priority - Employee washed hands with no soap. Observed employee washing hands with no soap. **Warning**
35A-10-4    High Priority - Live, small flying insects in food storage area. Observed approximately 10 small flies in dry storage area and three in men's restroom and the two in women's restroom. **Warning**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed rice on counter in bowl at 74°F. Observed bean sprouts at 80° on counter top. Operator stated they were from approximately 11:00. Stop sale on sprouts. **Warning**
08A-05-4    High Priority - Raw animal food stored over ready-to-eat food. Observed raw beef and chicken stored uncovered over uncovered sauces in walk in cooler. **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed two live roaches on cookline. One large roach under wok and one running across line from wok area to cooler area. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 8 fresh rodent droppings on canned goods on shelf in kitchen. Approximately 6 new droppings on dry storage rack. Approximately 41 old and new droppings on floor next to walk in cooler in dry storage area. Approximately 7 fresh droppings on clean pans on speed rack. Observed 1 fresh dropping on handwash sink. Observed 11 fresh droppings under three compartment sink in kitchen. Observed 3 fresh droppings on top of food storage container in prep area. Observed 6 old droppings in dining room in front of table closest to bathroom. Observed approximately 60 droppings on top of dry storage shelving. Observed more than 200 droppings on top of walk in cooler with an overwhelming odor of urine. The ladder used to look on top of cooler had 6 fresh droppings. **Warning**
01B-14-4    High Priority - Shell eggs in use or stored with cracks or broken shells. See stop sale. Observed shell eggs with broken shell stored above cooking grease uncovered. **Warning**

53B-13-4    Intermediate - Proof of required state approved employee training not available for some employees. To order approved program food safety material, call DBPR's contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. No current employees have training. Two books are available, but no proof of any training for employees. **Warning**
