Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-24-4    Basic - Hole in wall. Kitchen and rear prep areas.
10-24-4    Basic - In-use knife/knives stored in crack between equipment and wall. Knife in gap between wall and piping near HANDWASH sink at cookline.
05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. At 3 cookline coolers.
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. At ladies restroom.
14-40-4    Basic - Nonfood-contact equipment in poor repair. Rusted bottom shelving on prep table near the walkin cooler.
14-29-4    Basic - Nonfood-grade plastic to-go/thank you bags used in direct contact to store food. Used to store chicken at chest freezer.
29-08-4    Basic - Plumbing system in disrepair. HANDWASH sink at cookline missing drain pipe, leaking onto floor.

22-41-4    High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. 0 ppm chlorine.

01D-01-4    High Priority - Nonexempt fish offered raw or undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded. Tilapia used for ceviche.
35A-09-4    High Priority - Presence of insects, rodents, or other pests. 5 lizard droppings in dry storage area.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 3 live roaches in front counter area, 31 live roaches in soda box storage area, 7 live roaches at cookline coolers , 1 live roach in dishwashing area, 21 live roaches in gaskets at FRIGIDAIRE freezer near walkin cooler, 3 live roaches in shelving unit near 3 compartment sink and 4 live roaches by chest freezer in dry storage area.
