A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Dead roaches on premises. Approximately 2 on wall alone cooks line. Two on glue trap in dry storage area. Approximately 4 by coke machine, one in drive thru window area. THIS VIOLATION MUST BE CORRECTED BY 6/20/15 **Warning**on 6/20/15 approximately three dead under reach in cooler. One on wall at cooks line.
Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Kitchen area. **Repeat Violation** **Warning**
Basic - Outer openings not protected during operation and vermin and/or environmental cross contamination present. Hole in ceiling and around area where ceiling meets the wall, back storage area. **Warning**
High Priority - Roach activity present as evidenced by live roaches found. One on wall By prep area, one on wall by clean dish rack. Approximately Two on wall by hand sink cooks line. Approximately Two on wall in storage area by walk in freezer. Approximately Three on wall in dry storage. THIS VIOLATION MUST BE CORRECTED BY6/20/15. **Warning** on 6/20/15 approximately three under reach in cooler cooks line, one on wall by hand sink, one in hand sink, one on service window cooks line one on wall by hood vent cooks line, one on wall by clean dish rack.
Intermediate - No chlorine chemical test kit provided when using chlorine sanitizer at three-compartment sink/warewashing machine. **Repeat Violation** **Warning**
