Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

22-45-4    High Priority - Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. (Observed dish washer wash and rinse several metal buffet line pans without sanitizing. Only the wash and rinse bays are set up. Operator had employee run pans thru the dish machine.) **Warning**
41-05-4    High Priority - Pesticide/insecticide labeled for household use only present in establishment. (Raid can near wok station in back kitchen) **Warning**

35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. (21 on lower prep table shelf of back kitchen prep table. Prep table has open pan of raw skewered meats in prep phase. Observed 2 live roaches on the base of the slicer which is on a separate table next to and contacting the above stated prep table. 1large roach with an attached egg sac observed crawling on the table brace below the slicer. Roaches observed on the wall under the hand sink/3bay prep near the prep table. Possibly the same ones sprayed off the table. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Inaccessible droppings. Aproximately 5 in the corner behind the hot water tank in the dish wash room. 4 high on walls in same area **Warning**
